Our skilled nurses are completely dedicated to meeting the medical needs of our patients. Our nurses conduct initial patient assessment visits. They develop and implement patient home health care plans, working in conjunction with patient physicians and other healthcare providers involved in the patient’s plan of care. We strive to provide the most courteous and well-trained skilled nurses in the home health care industry.
Our skilled nurses must be currently licensed by the Board of Nursing in the State of Missouri, and we also require a minimum of one year of hospital, nursing home or public health nursing for all members of our nursing staff.

Our Home Health Care Assessment Process:
When a patient is interested in home health care for themselves or a loved one, a Med Plus Nurse is on-call 24 hours a day, seven days a week to assist with urgent medical questions or situational issues. We will initiate a complete phone assessment during normal business hours and evaluate the most appropriate home health services for the patient’s unique needs.
Once we have obtained all the necessary information, Med Plus HealthCare will conduct a Case Conference to determine and recommend a specialized treatment plan for the patient. Our Intake Coordinator follows up by contacting the patient’s physicians and other health care providers to receive their medical orders and to coordinate our services directly with their Plan of Care. We also contact and verify the insurance provider/payer source (i.e. Medicare, private insurance, etc.) and obtain treatment authorization when required by the policy.
When the insurance has been confirmed and authorized, A Med Plus Registered Nurse is sent to the patient’s home to complete a full physical assessment. (The RN will phone the patient beforehand to arrange the visit day and time frame.) If it is determined that additional medical services are needed, the Registered Nurse will then communicate with the patient’s doctors or other health care providers to coordinate the appropriate care.
