Med Plus HealthCare wants to make it easy for your office to refer your patients to us for home health care services. Our goal is to reduce paperwork and increase collaboration on patient care. If you have a patient that could benefit from our quality home health care services, please send the referral using one of these easy methods:

ALLSCRIPTS – for an efficient, secure, electronic-based transmission of web-based, health care records about a patient case in a hospital or clinical setting. Allscripts will transmit to us 24 hours a day, 7 days a week, including Weekends and Holidays.

FAX – during normal business hours within a typical M-F work week.

E-MAIL – to our secure inbox info@medplushc.com at your convenience. A Med Plus Intake or Marketing Specialist will contact you during normal business hours to review your patient needs.

TELEPHONE – to a local Med Plus HealthCare office to have a conversation about your patient needs, to quickly process your referral and schedule a clinician visit and patient assessment. Med Plus specialists are available during normal business hours, but skilled nurses are on-call 24 hours a day, 7 days a week.
